Farrar, Straus & Giroux, 1965-01-01. Hardcover. Very Good/Good. 8x5x1. Very good in good dust jacket. Jacket is not priced so presumably book club edition of some sort, although stated second printing so a little odd there. Binding is tight. Jacket is bright with just a touch of shelfwear, some fading at the spine, and a tear at the bottom edge of jacket front. Will ship (and look nice) in a quality clear Brodart cover.
