San Marino, California: The Huntington Library, 1967. First edition. Hardcover. Very Good/Very Good. Four pp. facsimiles. This was Mayhew's copy, and is signed and dated by him on the front flyleaf above a handwritten list of corrections to be made to the text. Original green cloth binding, with gilt titles. Some mild browning and wear along the extremities of the dust jacket; otherwise a crisp, very good copy.
