Quarto broadsheet, printed and completed in manuscript on recto and verso. Signed by Crews and Massengale on recto and verso. Toned uniformly. Very Good. The verso is the affidavit of Crews, signed and sworn to before Massengale, attesting his entitlement to the funds. The printed 'Atlanta' location has been replaced by the manuscript 'Knoxville.' Henry C. Crews [1836-c.1874], son of Dempsey D. Crews [1806-1892], enlisted with Florida First Cavalry Volunteers on January 15, 1862, assigned to Company K. He participated in engagements at Chickamauga, Siege at Chattanooga, Atlanta, New Hope Church, Jonesboro, Franklin-Nashville Campaign, and others. He was shot in the pelvis on November 30, 1864, at Franklin, transferred to the U.S.A. Hospital No. 11, and officially taken as a prisoner several days later on December 17, 1864.
